WHERE ARE YOU LOCATED?

We are located at 123 E Johnson Ave, Suite 4 in Chelan. We are on the bottom floor of the KOZI Radio Station Building next door to Washington Federal Bank.  **We are not affiliated with Lake Chelan Health Physical Therapy.**

WHERE DO I PARK?

We have limited parking next to our building, but there is additional parking on Johnson Avenue, behind City Hall/Sherriff's Office and across Johnson Avenue in the Pay Lot. Click here for additional information and a map of the area.

WHAT SHOULD I WEAR TO MY APPOINTMENTS?

Shoes: Tennis Shoes/Walking Shoes/Running Shoes; that are comfortable and sturdy enough to exercise in.

Clothing: Wear clothing that allows for easy access to the injured area(s) of your body that are also comfortable to exercise in (i.e. shorts or loose sweats for leg injuries and tank tops to access your shoulder or neck).

What To Avoid Wearing: Sandals, Flip Flops, Slip on Shoes with no Heels and High Heels.

WHAT SHOULD I BRING WITH ME TO MY FIRST APPOINTMENT?

The following are things you should bring with you to your first appointment with us.

  • Insurance card(s) *We need to have your physical card(s) to scan them front and back into your file.*

  • A valid picture ID, Driver’s License or VA ID

  • A Doctor’s Referral/Prescription  **Many insurances require a referral for physical therapy from your physician.**

  • Any chart notes, XRay Reports, MRI Reports pertaining to your current case (if available)

  • Any physical therapy notes if you were seen at a different facility (if available)

  • If under the age of 18, a Parent or Legal Guardian must attend your Initial Evaluation with you.

  • Someone to interpret for you, if needed, as we do not always have bilingual staff available.

WHAT IS YOUR NEW PATIENT/RETURNING PATIENT  PROCEDURE?

When a new patient or returning patient schedules their first appointment for a new case, they are scheduled in a New Patient/Case time slot for an Initial Evaluation. These time slots are either at 8:30am or 1:00pm so that we have our New Patient Room available for them. We ask that you arrive 15 minutes prior to your scheduled appointment time so we can complete intake forms, etc.

HOW LONG WILL MY APPOINTMENT LAST?

Initial Evaluations can take at least 90 minutes to two hours to complete, depending on the case. All other scheduled appointments can take at least 60 to 90 minutes to complete. We do our best to stay on schedule so that you will not have to wait. As a courtesy to others, please arrive 10 minutes early to your regularly scheduled appointments.
